This has been going on for a long time now. My cold starts are perfect no hesitation fires right off. When I start the truck after work in the evening or whatever when it has been warm I fire it up and have a rough idle for like 10 or 12 seconds. Also accompanied by blue smoke. WhT the heck is the deal? Iv been putting up with it for a while but I worked on a 24 valve at the shop the other day that started and ran way smoother than mine.

Ummm that is part of the reason I pull the head and had it reworked. I had failed valve seals. Valve guides start to fail and valve wobbles and the seals get damaged.

Mike's pictures show you what to look for.  Valve stem seals are about two hours to replace.  53 blocks are only really an issue if you start it cold and beat the piss out of it before it's warmed up.  Most problem 53 blocks have already cracked anyways.  It's been 17 years since they were cast.  These trucks are a pain in the *** but these issues you're thinking of selling it over are general maintenance and upkeep.

Well I never ever tag it when cold and rarely when hot. Makes me think the P.O. must have ran it pretty hard. I know he towed a good bit and did long hauls. I want to be sure it’s balve seals that are the problem what are other symptoms?

Blue smoke is oil.  It's either valve seals, bad head gasket, bad rings, or worse.  There are no other symptoms.  Cold oil gets in the cylinders and can't combust like the fuel does, and so you get the rough idle.  Easiest is to pull the valve cover, dig around in the oil for parts of the seals, and check the seals visually.  It's a 10 minute job.

If my seals are bad that means my guides are bad to right?

Oh boy I’m crapping my pants now. Lol. I can’t afford any expensive repairs right now.
