2004 3500 Manual Trans Conversion (APPS)

I'm attempting to use a 05-06 pedal mounted APPS. I bought the Timbo Kit and have it wired directly to the ECM.

This is the wiring diagram I came up with:
 

APPS Position   Description         ECM Location

#1                    5 Volt Supply      #33 (ECM C2)

#2                    APPS 2 Signal     #16 (ECM C1)

#3                    APPS 2 Return    #48 (ECM C2)

#4                    APPS 1 Return    #33 (ECM C1)

#5                    APPS 1 Signal     #14 (ECM C1)

#6                    5 Volt Supply     #42 (ECM C1)

However I am getting P2121 & P2128

I have tried two different pedal apps assemblies (APS312)

I have done the re-learn procedure over a dozen times.

ECM has been calibrated to a 2004 Manual calibration.

I have also attached two data log pictures. The first is with throttle off and the second is with throttle fully pressed.

Not sure if I am missing something or doing something completely wrong.

Any advice would be greatly appreciated. Thanks in advance!

Problem has been resolved.

Switched #2 & #5 at the APPS connector from the suggestion of another forum member (cumminsturbofreak).

That fixed my issue.

Thank you for the article Mopar1973Man. If this hadn't worked then I was suspecting a faulty ECM
