Is it pretty common for these trucks to not come equipped with overdrive?

As far as I know They all came with OD from 94 on with the release of the 47RH. The first gens 89-93 had a 727 in them with no OD.

I had a 93 with an auto. It had an overdrive and on the dash was a push button to shift back into third. I am not sure what year they started putting overdrive in the Cummins? My 99 came with an auto and it too had overdrive but the button was on the shifter instead of the dash.
