Need to be schooled in Big rig driving...
Thats the only big truck trannys I ever drove. Other than the first couple months learning them, they wer pretty easy for me. I do remember having to pull over and stop so I could start over quite a few times in that first couple months. it was to say the least . We had a nice GMC straight flatbed and an old raggedy assed Ford gas burner pulling a 1957 Great Dane trailer that had more irom in the frame than you could put on the trailer. Did not have much trouble with the tranny, stopping was the real adventure.

Mine acted the same when my return line tee and bajo on the head were leaking. Still does it every now and again when the fuel tank is real low. I bump the starter and let the lift pump cycle once and it starts right up. sounds like you got a little air coming in somewhere.

Ethanol Side Effects
Many years ago I thought ethanol form corn was a good idea. I have long since changed my mind when i saw how it was affecting our food supply. It floors me that more people cant see whats happening and I am just speaking of prices at the counter not effects from farming additional land. I have to agree with the use of non food agricultural products for fuel for the car and leave the food for the life forms that need it.

How bad has it driven up feed costs? And how far has feed cost driven up the price of meat? Maybe you save on the cost of fuel a little, but spend any savings buying food. A vicious circle if you ask me, and not a real solution.

Awapping VP44 & side cover gaskets
Hopefully I will be in there in the next couple months and see whats in there. My AD is on the frame rail so maybe none of that is in the way.

I will be doing this in the near future. Has any one tried adding a breather to the 24v tappet cover? Is it is a casting or just a stamped metal cover?

The rotor button was kind of strange. It looked 2 thin layers of metal with a synthetic layer sandwiched between them. The last rotor button I messed with was solid.

My 02 does not have that feature. I never noticed any second gen coming at me with low power headlights burning.

Not making boost as of recently.
I am not the electronics wizard here by any means, but from the codes your alternator might be putting out to much juice. Thats where I would start checking. It could be creating enough havoc to cause the engine to defuel and that will lower boost. Go over to the articles section and look under 24v at the OBDII error codes. I cant seem to link anything anymore.

18 around town in a 8K truck is asking alot. I just dont see it happening. 22 highway, maybe. I have seen almost 21 once, But i was keeping my speed 65 max. As far as Smarty settings, I know nothing about them.

95 Honda accord
Well, I made it home and it crapped out on him. I went over to his house and sure enough no spark. So we took the distributor cap off and The contacts the rotor button sends power to were all pitted up and the rotor button was pretty well gone also. Replaced both and it fired right off. Appreciate the help and all is well.

ECM Discussion....
As one of the ones who have had a bad experience with rebuilders and weeks waiting for a rebuild of the rebuild, I finally got one from Dodge. This happened 2 years ago and I was under the impression that remans were all you could get then. Could be wrong. Personally if mine were to go out again I would go with the Dodge, or Cummins, who ever had the best warranty and or price. I would agree with electrical spikes and heat would be just as bad for the ECM as any other electronic component. Would the wiring harness be long enough to allow you to move it very far with out having to modify it? Does anyone know how hot it gets on that side of the engine?

Ouch!! Also sorry to here of your troubles. I have not experienced frost heaves, but have seen alot of heaves from heat over the past couple of years. It does the same thing, buckle the road up and the asphalt can launch your vehicle. You have our thoughts and prayers here for a safe journey and a special one that the kids think it is all an adventure. As a father of 3 and a gran daddy of nine, I can feel your pain.

Vacuum pump - Rebuild time!
I cant seem to find the time to fix mine. Hopefully that is coming real soon. I have been leaving my mark any where I park for the past year and a half. Tappet cover is leaking, I think the front main seal is leaking and the it appears the vacuum pump seal is going now. I have already had to add 2 quarts of oil and I am only 4k into this oil change. That is about double what I have been seeing of late. So the Valdez will have to cruise on a little while longer.

Your numbers are pretty close to what I get. 14 around town and about 19 highway. That is empty at about 7.3k#. I have personally never seen 18 mpg in town since mine was new. 9 psi of fuel pressure is to low. I am going to assume you do not have a FP gauge. Get one asap. Yes you may have damaged the VP from low pressure. But it sounds like it is still working fine for now. It might last 6 months or 6 years. Get the FP problem fixed sooner rather than later. Before i had a FP gauge I had problems and found my pressure to be close to non existent. I fixed the problem and the pump went 3+ more years before I replaced it There are many schools of thought on proper pressure. I personally run 19 at idle and 15.5 at WOT. Anything around 18 to 20 at idle and 15 to 17 at WOT is very acceptable. How old is your AD? .

Possible Dead ECM?
When mine went I had the revving you experienced. You could stand by the truck looking to see who was in the cab playing with the throttle. Mine never smoothed out though and was undriveable. My WTS functioned normally but nothing else did. I also had a long list of codes, mostly sensor high and low voltage warnings. The WTS malfunction is definitely ECM in nature, but I agree with Mike on checking more before throwing an $1800 part at a problem before you are pretty well sure thats whats wrong.
