High idle kit problems
OFF - Both sensors are seen as normal. MPG - The IAT sensor is fooled. ScanGauge II should report IA 143*F 6 CYL - The IAT and ECT are both fooled. The ECT fooler makes for the extended hold feature of the high idle. ScanGauge II should report IA 26*F and WT 123*F 3 CYL - The IAT is fooled. ScanGauge II should report IA 13*F

Wait to start, waits only when cold
Good ol' Yankneck696 usually suggest to go over all the connectors with dielectric grease and then cycle each one 3 time to clean the contacts and also work the grease into them.

Double check your grid heater relay wiring. The trigger wires are just push on type.

Exhaust brake
I've got the Jacobs Exhaust brake and love it. But I'm still running 3" exhaust too. I would opt toward Pacbrake more so for what you doing.

In one smooth movement put the cat in the toilet and dose the lid. You may need to stand on the lid. 4. At this point the cat will self agitate and make ample suds. Never mind the noises that come from the toilet, the cat is actually enjoying this! 5. Flush the toilet three or four times. This provides a ‘Power-Wash’ and ‘Rinse’. 8. Have someone open the front door of your home. Be sure that there are no people between the bathroom and the front door. 7. Stand well back, behind the toilet if possible, and quickly lift the lid. 8. The cat will rocket out of the toilet, streak through the bathroom, and run outside where he will dry himself off. 9. Grid heaters not working since high idle cold start enabled at dealer
Some times you get a condition where the rear axle is spinning but the front axle isn't moving then the ABS computer gets confuse and trips the P0500 code because how can half a truck but motionless and the rear half is moving 25 MPH. ABS computer can't compute that.

Carpet or rubber floor mat
I tend to prefer carpet over rubber. The rubber flooring typically end up rotting the floor board out of the truck.

Check your wiring for the grid heaters. See if the fusible links have failed. See if the any P0380 or P0382 codes.

Yeah but if the software never boots up in the ECM then there is no till the software is booted up. The first few instructions is to check IAT temp and figure out how long to run the grid heater.

Miss at all RPMs
Even talking to Ed he's starting to think there is something wrong valve train wise but not sure either. But is odd how the compression and other test show its in good shape.

DTC codes - P0251 oil pressure rationality
Monitor & Set Conditions Oil pressure rationality is monitored with ignition on and engine off. Time for a oil pressure sender.

Personally in vendor that deals with Industrial Diesel. (CPP or Vulcan Performance) All Industrial Injection pumps come with a NEW PSG for a low price already. Why I usually do my own work whenever I can....
I tend to agree the axle stub does look like its taken water in a few times. The grease don't even look good. I typically use the red high temp grease.

This is why I'm posting up on the site I remember talking to gent that was in the Idaho state and did masonry work. I know there is other people like JAG1 on other that do home repairs and such. I admit I've got a strong abilities and then I've got my weak ones too. Blocks and walls are not one of my strong ones.

I'll say Tom's right on the P0234 code. As for the transmission shift issues I would say it governor pressure solenoid and sensor. (Educated guess) I'm also not very good with 47RE trans. But welcomee to Mopar1973Man.Com

Same here I rather do the work myself. So far if I screw up I've only got myself to blame. As for someone else screws up now you got to prove it and hopefully get it resolved.
